A spray-type adhesion barrier with nozzle applicator

When tissue is lost or damaged during surgery, etc., a phenomenon known as postoperative adhesions may occur during the healing process. In this process, organs and tissue surfaces that are normally separated become connected.
Terumo has developed AdSpray, a spray-type adhesion barrier as an intervention to reduce postoperative adhesions.

This product forms a hydrogel which acts a physical barrier. Terumo has adopted NHS (N-hydroxysuccinimide)-modified carboxymethyl dextrin polymer. The powder containing this main ingredient forms an acidic solution. When mixed with an alkaline solution, a crosslinking reaction occurs that forms intramolecular ester bonds, and a gel is quickly formed.
Starting from this gel technology, Terumo moved ahead with in-house development, including the development of an applicator, and brought it to market under the concept of ‘an easy-to-use spray-type adhesion barrier.’
Since the product’s launch, it has been used in a variety of abdominopelvic surgeries, as well as in various approaches such as open surgery, laparoscopic surgery, and robot-assisted surgery, in order to reduce the frequency, extent, and severity of postoperative adhesions.

Excellent biocompatibility

Many adhesion barriers are made from bioabsorbable materials and are designed to be absorbed by the body after the period of isolation between injured tissues. AdSpray uses dextrin compounded from a substance derived from corn starch.

Hydrogel that can be applied to complex surgical sites

Two solutions pushed out of the spray applicator are mixed uniformly by gas in the applicator tip, and sprayed in a mist around the surgical site. The two sprayed solutions quickly form a gel over the damaged area and form a barrier, making it effective even when sprayed along unevenly shaped areas.

Excellent accessibility suitable for laparoscopic surgery

AdSpray’s nozzle can be inserted through a 5mm port, making it possible to approach the target surgical field in various abdominopelvic surgeries. It can be used regardless of surgical approach, including open surgery, laparoscopic surgery, and robot-assisted surgery.

Visible even during endoscopic surgery

Microbubbles are generated at the tip of the nozzle during the mixing and spraying of the two solutions, resulting in a whitish gel that is highly visible. The sprayed area can be confirmed during surgery without the use of colorants.
